I took 0.5 dexamethasone (which I think is equivalent to 25mg prednisolone) for 2 months. The only side effect I had was the facial hair - which would not have been my top choice if I got to choose a side effect!

When coming off them, I took the dex on alternate days for one week before stopping completely. During my dex-free days, I was lethargic and had headaches.

Treatment of NK cells

•  This very new scientific research that enables us to establish a link between recurrent miscarriage and the abnormal behaviour of the mother's immune system may sound unfamiliar and complicated to some women. Because it is such a cutting edge science, it is quite possible that your GP or even other specialist consultants may not have heard of such a connection, and be sceptical about it's importance.

•  The good news however is that we can do the diagnostic tests here in our clinic, and if the results are positive for elevated NK cells, we can also offer you an extremely effective, safe and inexpensive treatment, after which the chances of a positive pregnancy outcome are increased to 80%. The main component of the treatment programme is the prednisolone. Aspirin and heparin are usually given to help out in eradicating the undesirable effects of the NK cells in case they are not all suppressed by the prednisolone. These drugs are safe in the doses we prescribe and we will discuss any worries or possible side effects with you'

Safety of drugs and possible side effects'

Prednislone : does not pass through the placenta easily and is also broken down by enzymes in the placenta so that the fetus is exposed to only trace amounts. 

Additionally, the body produces the equivalent of 8 mg per day of this corticosteroid. When indicated, Prednisone should be started prior to conception. 

Maternal side effects are mostly reversible and include weight gain, mood swings, hair fall or growth, increase in blood pressure and blood sugars, sleeping difficulties and energy and skin problems. 

Heparin : it is a large molecule and therefore does not cross the placenta and is harmless to the fetus. It is a daily injection which may cause slight bruising, but otherwise safe to the mother. Rarely, as any other drug you may develop a drug allergy, which may lead o change of brand. 

Aspirin : is safe for the fetus. If you have asthma or stomach ulcer you need to inform me prior commencement of the programme. 

Three-Fold Approach

His approach to this problem is three-fold. First, in addition to just the numbers alone, his team looks at how aggressive the NK cells are. Highly aggressive NK cells are a red-flag. Second, he prescribes a simple, low cost steroid treatment as a counter-balance. The steroid prescription itself can cost less than $25. 

Third, and this is what distinguishes him in his field, he prescribes steroid treatment before a patient is pregnant. 

"My theory is that any miscarriage starts at the time of conception, the trick is to start treatment before a woman conceives," he says. "[Some people] say. 'If we see a baby's heart then we will give you the treatment.' In my opinion you are playing catch-up and that is a very dangerous game." 

Dr Shehata's treatment, using steroids, is pioneering because it starts before conception and the doses involved are higher than previously used. 

Ms Baker fell pregnant for an 18th time but her troubles were not over. 

During the pregnancy it was discovered she was diabetic and the high sugar levels caused by the steroids resulted in another miscarriage. 

The doctor was able to adjust her levels of insulin and the next time her pregnancy was successful. 

Hi flossy ... I've been treated for high NK activity (& anti-progesterone antibodies plus APLS) - I was diagnosed at the ARGC in London.

My pg history is :-

1 - miscarriage 8 weeks

2 - live birth 36 weeks, pre-eclampsia

3 - miscarriage 5 weeks

4 - miscarriage 12 weeks

5 - miscarriage 6 weeks

6 - miscarriage twins - 1 at 7 weeks one at 14 weeks - private treatment IVIG/clexane/aspirin 

7 - live birth 38 weeks - private treatment - cyclogest & presdnisolone until 14 weeks, aspirin & clexane throughout.

8 - miscarriage at 17 weeks - NHS treatment aspirin only  

9 - live birth 38 weeks - NHS treatment - 25mg prednisolone/day until 14 weeks, 2x cyclogest suppository daily until 14 weeks, & 150mg aspirin & 20mg clexane/day throughout. 

For me it seems to the the prednisolone that makes the difference. My antibody & NK levels are borderline btw ... I managed to get my NHS consultant to treat me aggressively on the basis of my history of losses.

positive test - begin aspirin, clexane, cyclogest & prednisolone (I have a supply at home when I'm ttc). I take high dose folic acid when ttc that continues.

5 weeks - blood tests for platelet levels (because of clexane), book scan for 6 weeks.

6 weeks - scan - see consultant continue meds if gestational sac present.

7 weeks - re-scan - if heart beat present continue meds - 25mg pred, 20mg cleaxane, 150mg aspirin, 2 cyclogest suppositories (can't remember dose is it 150mg?)

weekly scans until 12 weeks then start weaning from pred at 12 weeks - off completely at 14 weeks (stop cyclogest also).

Then scans x2 weekly until 20 weeks - then have fetal med anomaly scan - they check for cleft palate (potential risk from steroids) & have first of placental function (doppler) scans.

Scans continue every two weeks until birth (with my last pregnancy there were signs that the placental was crapping out at 28 weeks so I had weekly scans) & doppler studies done monthly. I have elective section booked for 38 weeks but have CTGs done 3x week in day assessment from 35 weeks. 

The biggest worry that my consultant had was the clexane - she's advised no more pregnancies (I think another pregnancy would kill me because of the effect on my bone density).

I don't know about testing - I have been tested for everything repeatedly and have never had any real answers - I got to the point that I just wanted to try something & was happy to accept the risks of some unproven treatments.

I got a prescription from Dr Gorgy from a telephone consult. He wanted to know what my blood results were and what had happened with previous cycles and any other results I had. From this he said I would need intralipids, steroids, gestone, clexane and aspirin, I was also taking high dose folic acid, B12 complex and Vit D. He emailed me the script and also sent one by mail. My GP re-wrote the script for me and I used Point of Care to give me the intralipids. After 4 IVF's and 1 FET we now have a beautiful baby girl and I am convinced it was the intralipids he prescribed that did it for me as I was on all the other meds in all my previous cycles.
