大部分女性会在排卵期时内膜增厚到7mm或者更后。通常我们不是很确切知道为什么内膜会很薄，但研究发现主要与前期的子宫手术有关（比如引产）。内膜薄的女性可能会有子宫内膜疤痕，这可以通过宫腔镜检查来确认。可以帮助子宫内膜增厚的药物通常包括经阴道补充的雌激素和西地那非，维生素E和己酮可可碱。 在这些药物中，雌激素可能是最有效的，而其他药物的效果只在小范围的研究中被证实。当然，药物的帮助总是有限的。通常我们在周期开始前，会让患者进行一个尝试治疗，看看药物是否对子宫内膜厚度有帮助，确有帮助时才进行IVF。如果女性的子宫内膜始终都无法增厚，那么在美国，她可以考虑使用第三方辅助妊娠。对于37岁以上的女性，也会考虑第三代试管技术（对第5天或第6天的囊胚进行染色体分析），增加着床成功率。 通常，37岁的女性，她的囊胚中约有50%的概率存在染色体异常，这也是着床失败的主要原因。

Most women have an endometrial thickness of 7 mm or more around the time of ovulation.  Often we do not know why the endometrial lining is thin, but a common cause can be a history of previous uterine surgeries.  Women with a think lining may have scarring of the endometrium.  This can be evaluated through a surgery called hysteroscopy.  Medicines that have been used to try to make the lining thicker include vaginal estrogen, vaginal sildenafil (Viagra), Vitamin E, and pentoxifylline.  Of these different medicines, estrogen is the most likely to be effective, and the other medicines have been evaluated in only very small medical studies.  Unfortunately, these medicines are usually not very effective.  We sometimes will use some of these medicines in a trial month of treatment before IVF to see if they help thicken the lining of the uterine cavity.  Women who have persistently thin lining and who are infertile can consider using a surrogate to carry the pregnancy, if the couple is treated by a fertility clinic in the US.  Usually, this is associated with third generation IVF (genetic testing of the embryos) if women are 37 years old at the time of treatment.  At age 37, approximately ½ of the Day 5 and Day 6 embryos are genetically abnormal.
